Registration Process
Once a student has accepted their seat at Cherokee Classical Academy, they will be required to complete registration online with the necessary documentation needed for public school enrollment. The deadline for document submission will be clearly communicated in the email used to offer the seat.
Proof of Parent/Guardian ID:
Drivers license
State identification card
Passport
Other government-issued photo identification
Proof of Address (Residency):
Provide one (1) proof of residency from BOTH Column A and Column B
Column A: Proof of Ownership/Residence
Non-contingent sales contract
Current lease/rental agreement
Current residential property tax statement or bill
Current warranty or quit claim deed
Current home purchase agreement
Current homeowner’s insurance policy
Column B: Utility Proof
Current water bill
Current electric bill
Proof of Student’s Age:
Certified birth certificate (Parents can order one HERE)
Hospital-issued birth record or certificate
Valid driver’s license (if applicable)
Passport
Adoption record
Religious record signed by an authorized official
Additional documents are required to complete student registration and must be submitted no later than July 1, 2025
Social Security Card (or signed waiver as provided for by OCGA 20-2-150(d).
Immunization Records
Ga. Health Dept. Form #3231 (Certificate of Immunization)
Form #3300 (Vision, Hearing, Dental, and Nutrition Screening)
Exceptions
Notarized Affidavit of Religious Exemption (Ga. Health Dept. Form #2208).
Medical exemption must be noted with expiration on Form #3231.
Medical or Specialized Services Documentation (if applicable)
Allergy documentation
504 Plan
Individualized Education Program (IEP)
ESOL
Previous School Records
Report card or official school transcript
Withdrawal form from previous school
Discipline records (required for students enrolling in grades 7-12)
Where applicable, proof of eligibility for economically disadvantaged status and financial aid:
Most recent income tax return (2023), or
Current award letter or card for SNAP, TANF, WIC, Medicaid, or PIN
Special Circumstances: Residency Affidavit (March 1, 2025)
If the student’s family resides with another individual, additional steps are required.
The person with whom the family is residing must provide:
○ A valid ID
○ Two forms of Proof of Residency (one from Column A and one from Column B).
Student’s family must provide a notarized third-person affidavit of residency including:
○ Signatures of the person with whom the family is residing and the parent/guardian.
○ Signature of the apartment manager, if applicable.
Student’s family must provide one piece of business mail in the parent/legal guardian’s name with the residence address (must be submitted within 30 days).
